INSTRUCTIONS AND DOWNLOAD INFORMATION
The following documents and forms are available for you to download and make your registration process a little easier. Just click on the form that you need, and it will pull up. You need to completely fill in a spaces with your information.
The forms are in Portable Document File (PDF) format which will make downloading and copying very easy. You will need ADOBE ACROBAT READER to view these documents. Most programs come with ADOBE ACROBAT READER to view 'Read me' files. If you do not have this program, you may click here ADOBE ACROBAT READER to download a FREE copy of this program. You will need approximately 5.4 MB of disc space to install. It will take less than a minute to download with a cable modem and from 15 to 45 minutes to download with a phone line modem depending on your computer. NCYSA MEDICAL WAIVER FORM (PDF)    Please fill out this form completely. Do not skip any lines and make sure you put the legal guardians initials in the WAIVER BOX at the bottom of the page. Form MUST BE NOTARIZED before mailing to SCW.

SCW PLAYER SIGNUP FORM (PDF)  Please fill out this form completely. Please print legibly so that the information can be transferred to our database correctly.
